Medicare Part A is premium-free for most people. You can lose your Medicare Part B coverage if you don't pay your premium ($104.90 per month in 2014 - the premium can be higher for higher-income individuals). Most people will have their Part B premium withdrawn from their social security check or bank account directly (electronic withdrawal) so they don't have a lapse in coverage.
