The simple answer is, because Congress has no idea how to manage a health insurance system. Medicare is not now, and never was, run like a business.
It was a political vote buying scheme.
As long as DC has the ability to print money and hand out "goodies" without regard to balancing the books, Medicare and other programs will continue to be underfunded.
