   You are eligible to apply for medicare during a span of 7 months beginning 3 months prior to the month of your 65th birthday, including the month of your birthday and the 3 months following your birth month. Navigate to medicare.gov or the Social Security website, www.ssa.gov.  From either of these portals you can gain access to the necessary enrollment forms.
