In order to have Medicare you need to either be age 65 or older or you need to qualify by being on disability for longer than two years.

To get Medicare Part A at no cost, you need to have worked and paid taxes for 10 years (40 quarters). You will always pay a premium for Medicare Part B.

Most people are automatically enrolled in Medicare Part A at age 65. If you or your spouse have enough Social Security credits there is usually no premium charged for Part A.

Enrollment in Medicare Part B is elective on your part, although Medicare will usually enroll you in Part B as well.

If you currently are covered under an employer group health insurance plan you may delay enrolling in Part B without penalty.

If you have paid into Medicare through payroll taxes for 40 quarters and are drawing on Social Security at age 65, you will automatically be enrolled in Medicare. If you are younger and wanting to join Medicare, you do so by paying Medicare taxes. They are charged for self employed, employees, and government workers, so simply working at a legitimate job will get you on the road to receiving Medicare when you are age 65+.
