The insured person and the owner of the policy (if they are two separate people) must sign an application for life insurance, in most situations. If a separate person is paying for the policy, that person may have to sign authorization for bank account. The beneficiaries do not have to sign.

California is one of five community property states.  In addition to the insured signing the application, the signature of both the husband and wife are required if they own a policy where the beneficiary is someone other than the two of them.  For example, if I buy a life policy on my life and name my sister as beneficiary, my wife also has to sign the application.
