A life insurance urine test along with a blood test with larger policies are checking for two categories. The first series is looking for drugs, both illegal and prescription to test against medical records for abuse. Underwriting will test for nicotine and cotine, which stays in the system longer than just nicotine for tobacco use. The second set of tests are used to check for diseases like cancer, diabetes, heart disease, blood disorders, kidney disease, etc. These results are usually compared to the individuals medical records for their risk assessment.
Urine testing can give indications of kidney disease, but kidneys are involved in more than just renal disease. E.g. Protein in the urine can be a sign of uncontrolled diabetes. Urinalysis can evidence hypertension, or show that the applicant is not complying with treatment for hypertension (i.e. not taking their blood pressure medication as prescribed).
Urinalysis is also used to help determine whether the applicant uses tobacco or illegal drugs, or abuses drugs or alcohol.

The carriers are testing for diseases, such as diabetes, and kidney problems. But they are also testing for drug use, but legal and illegal drug use. If someone is taking a prescription drug and does not disclose that, the carriers wants to know, because the prescription would normally be to help a non disclosed illness.
