If you have an individual health insurance policy with an insurance company the contract is directly between you and the insurance carrier. If the coverage is through an employer group then the contract is between the employer and the insurance company, the covered employee does not have a direct contract with the insurance company and has a certificate of coverage outlining the coverage they have through their employer's contract but the employee themselves are not the contract holders.Answered on April 9, 2014flag this answer
