    I suggest that if you have a complaint or grievance with your health insurer, that you contact your State Insurance Department to file a complaint. Your State Insurance Department will be able to handle it, or at least direct your type of complaint to the right authority. Of course, these sorts of complaint filings are long and drawn out and can take anywhere from 90 days to 3 years. So, you might be better off trying to work with your insurer first and exhaust all of those procedures before going to the authorities. That way, if the insurer is being malicious or faulty, you have more evidence to support your complaint. And depending on the type of grievance you have, all insurer's have appeals and grievance processes to resolve issues and avoid any legal battle(or bad reputation backlash with consumers). If you have an insurance agent, take it to them first and have them help you file any paperwork with the insurer. If you don't have an agent, you can contact the insurer yourself. But, definitely exhaust those option first, then go to your State Insurance Department. If they are unhelpful, then you could always go to the federal Department of Health and Human Services and/or Consumer Protection Agency.
