Cosmetic surgery merely for the purposes of enhancing a person's physical appearance will be an exclusion under individual and group health insurance policies.
The possible exception to this rule would be in the circumstances where cosmetic surgery is required as a result of an illness or injury and is required to restore normal function (or as close to it as possible). Even in these circumstances the policyholder should expect that their insurance carriers will require medical documentation as to the medical necessity for this procedure and a pre-authorization from the health insurance carrier is a virtual certainty.
