You will need a claims form and the proper documentation from your physician defining your disability. The physicians office will have that form. If you are not covered by disability insurance whether individual or group and you have a claim for disability insurance through the Social Security Administration then go to http://www.ssa.gov/disabilityssi/ and follow those steps.
