There are five basic steps needed to file a disability insurance claim.
One - contact the insurance company that issued the policy, and obtain their claim form. Two - complete the section asking for identifying information (name, address, SSN, policy number), and sign the form. Three - have your doctor state the medical reason you are unable to work, and sign the form. Four- have your employer verify that you are no longer working, and sign the form. Five - return the completed form to the insurance carrier.
