    First, if you are filing a disability claim, contact your broker or agent if you have one. They should have the contact numbers and will be able to get you the claim form.

    If not, call the company where the policy is and tell them you need to file a claim. They will ask for your policy number and verify who you are. Then they will either email or mail a form out to you which you may be able to email back if not regular mail.

    It is a process but feel free to ask them questions so you are in the loop. It may take a bit of time before you start receiving your benefits.
