You may need to go through an agency who handles many carriers to find one. Two carriers that offer it are Assurity Life and Mutual of Omaha Life. If you use a life insurance agency that sells products from those companies, they can help you, if they understand disability insurance.Answered on August 3, 2013flag this answer

If it is offered through your workplace, that is usually your best price.
