		<description><![CDATA[One of the easiest ways to find out who your insurance carrier is for Medicare Advantage and/ or Prescription Drug Plan coverage is through www.Medicare.gov.

On the main page of Medicare.gov, click the button that says SIGN UP/ CHANGE PLANS and then select Check Your Enrollment from the menu.

Have your Medicare card handy.&#160; You&#039;ll need to enter your zip code, Medicare Number, Last Name, Effective Date for Part A, Date of Birth and press the Continue Button.

The next screen will display insurance carrier and plan information associated with your Medicare number.
