		<description><![CDATA[Through a Medicare Administrative Contractor (MAC) - private organizations that carry out the &lt;strong&gt;administrative&lt;/strong&gt; responsibilities of Traditional &lt;strong&gt;Medicare&lt;/strong&gt; (Parts A and B). They also handle durable medical equipment, home health and hospice claims.  MACs are assigned by state; a provider can check with CMS (Centers for Medicare and Medicaid Services) to find their applicable MAC.

New eligible providers complete and submit a form (CMS-460) to their MAC along with a Medicare enrollment application.  Each year providers who are currently enrolled in the Medicare Program have an open enrollment period (Nov. 15 - Dec. 31) when they can change their current participation status beginning the next calendar year on January 1.]]></description>
