Both Medicare and Medicaid are insurance programs provided by the government for the most vulnerable groups to illness and injury.  Both programs receive money from the federal government, but Medicaid is actually managed by the individual states.  Medicare is for those over age 65 and with certain medical conditions while Medicaid is for anyone with limited income.  
Individuals needing to sign up for Medicare must do so through the Social Security administration.  The Medicare website provides excellent information on the various benefits and additional coverage that may be available.  Those interested in applying for Medicaid can now do so through their state or federal health exchange website or call center.
