First, download the claim form from the web portal of the insurance company issuing the policy. Second, complete the personal information section. Third, have your doctor complete the physician's report section stating the medical reason why you are unable to work. Fourth, have your employer complete the appropriate section verifying that you are no longer earning an income. Fifth, sign the form and return it to the insurance carrier for review.
