    In most cases, you do not need to file a claim to Medicare, as doctors/hospitals are required to file these claims for you in a timely manner for Medicare-covered services and supplies. If, however, your doctor has not filed a claim, you can contact them and ask them to do so or call Medicare at 1-800-MEDICARE.

    If it is close to the time limit (12 months) and your doctor has not filed the claim, you can do so using Medicare's Patient Request for Medicare Payment Form (CMS-1490S).
