There are three limited scenarios under which a person can start Medicare before age 65. There are other qualifying criteria for each of these conditions. It is best to call your local social security office for details.

The first is people who are disabled and receiving Social Security Disability Insurance for at least 24 months. The second is people diagnosed with end stage renal disease who are getting dialysis or have had a kidney transplant, and are eligible for SSDI. The third is people who have been diagnosed with ALS - Lou Gehrig's disease.
