    The most common approach to file an appeal to a Medicare claim is to complete a "redetermination request form," and send it to the Medicare contractor at the address listed on your Medicare Summary Notice.  You should circle the items your would like appealed, explain in writing (with as much detail as possible) why you disagree with the original determination, and include as much information as possible to back up your case. An appeal will always have a much higher likelihood of succeeding if your doctor or other health care provider also supplies a written statement backing your position.
