Medicare does not pay for physicals unless they are for a specific complaint. The "Welcome to Medicare" exam is the only thorough exam provided at no cost, but it is a one time thing, and does not include blood testing unless warranted. Annual Wellness visits are provided at no cost. However, these are not nearly as thorough as a routine physical exams.

In the first 12 months that you have part B, you can get a “Welcome to Medicare” preventive visit at no cost to you providing the provider accepts Medicare assignments, be sure you tell the doctor’s office that you want to schedule your “Welcome to Medicare” Preventive visit.
You can also receive an annual “Wellness” visit to develop or update your personal health history. However, these visits are not as thorough as the “Welcome to Medicare” visit. Here again, you pay nothing for the yearly “Wellness” visit if the doctor or provider accepts Medicare assignments.
