If you are receiving Social Security benefits at age 65, you will be signed up for Medicare Part A and Part B automatically. When you get the packet in the mail, you will have a form that you can use to turn down Part B. At this time you can also opt out of Part A, but it is free and covers very expensive hospitalization costs, so you would be advised to keep it, or have very other good health insurance if you turned it down.
If you are not yet receiving Social Security benefits at age 65, it is not mandatory to apply for Medicare Part A or B at that time. However, it is again, advisable to at least get Part A.
