If you purchased the coverage and did not receive evidence of the coverage, binder, or a policy, contact the source of the insurance (if you know the agent or broker) and ask for a policy.  The declarations page(s) should set forth the name of the insurance carrier, the name of the insured(s), and the particulars of the contract.  If you did not purchase the coverage, some entity (like a lender/mortgage holder, homeowners association, Co-op, or Condo association, etc. with an insurable interest) may have purchased the policy.  You can contact this entity and pose your question.  In most states a homeowners policy must be in the name of the owner (showing insurable interest) and the home must be occupied by the owner (eligibility requirement for most homeowners policies.)
